Our work is fundamentally the provision of a range of care and support activities to vulnerable individuals to ensure their safety, optimum health and quality of life. These care activities that we provide are evidence-based, commissioned, regulated and externally monitored or inspected, to ensure its safety, effectiveness and responsiveness to the needs of the people we serve, in co-operation with our stakeholders and partners. Care Reviews: Fortnightly Case Management Meetings, Quarterly Case Reviews, Medical Reviews, and Annual Placement Reviews.
Care Recork Keeping and Communication: Care Needs Alert – Our automated system for care provision management, the recording of care delivery and the provision of timely notification, alerts, or communication with relevant stakeholders on issues or concerns about the service user, Care and Legal Record Management and the automated timely sharing of critical care records with professionals.
Practice Supervision: Regular staff support sessions, underpinned by an individual staff supervision contract.
